Registration

To enroll a new student, you will be required to provide the following documentation:

• Official Birth Certificate

• Illinois School Physical

• Complete Shot/Immunization Records

• Proof of Residency (See requirements below)

To verify your residency within District 118, you must provide three (3) pieces of information showing your current address:

1. Photo Identification (Required)

• Valid Driver’s License, State ID, or Photo ID.

2. Primary Residence Verification (Choose One)

• Homeowners: Most recent property tax bill (with proof of payment/Form 1098) OR Mortgage papers.

• Renters: Signed and dated lease AND proof of last month’s payment (canceled check or receipt).

• Trailer Park Residents: Letter from manager AND proof of last month’s payment.

3. Secondary Address Verification (Choose One)

• Utility Bill (Gas, Electric, or Water)

• Vehicle Registration or Insurance card

• Current Bank Statement

• Voter Registration

• Current Public Aid Card or Insurance Policy (Homeowners/Renters)

• Recent Cable or Credit Card bill

If a student’s change of residence is due to a military service obligation, the legal custodian may submit a written request to maintain residency as determined prior to the military obligation (Policy 7:60).

Required for all Pre-K, Kindergarten, 6th Grade, 9th Grade, and all NEW students. Illinois law requires these students to have a current school physical and up-to-date immunizations. Students will not be able to attend school until these are completed.

Examination Requirements by Grade

• Physical Exam & Diabetes Screening: Required for Pre-K, Kindergarten, 6th Grade, and 9th Grade.

• Lead Screening: Required for Pre-K and Kindergarten only.

• Vision Exam (by Optometrist): Required for Kindergarten and any student new to Illinois schools.

• Dental Exam: Required for Kindergarten, 2nd Grade, 6th Grade, and 9th Grade.

• Sports: All students participating in sports must have an athletic physical on file (updated annually).

Required Immunizations

Proof of the following is required before a student may start school:

• DTP, Polio, MMR, Varicella, and Hepatitis B

• Tdap: Required for 6th Grade.

• Meningococcal: Required for 6th Grade (after age 11) and 12th Grade (after age 16).
