• Health Requirements

    All students must have proof of proper immunizations and a physical before entering school; a lead blood screening for all pre-school and kindergarten students is also required. Eye exam is due by October 15 of the current school year for Kindergarten students and any students entering Illinois schools for the first time. Kindergarten students and new students to the district will need a court house birth certificate, and proof of residency. Kindergarten, second and sixth grade students must provide proof of dental exam by May 15 of the school year.

    All students entering, advancing, or transferring into 6th, 7th, 8th, 9th, 10th, 11th, or 12th grades will be required to show proof of receipt of one dose of the Tdap vaccine (combined tetanus, diphtheria, acellular pertussis) regardless of the interval since the last DTaP, DT, or Td dose.

    All 6th grade students must have:

    A new Illinois State Physical.

    Current and up to date shot record; new shots for 6th grade include a Tdap, Meningococcal (MCV).

    *Student are not allowed to start school until above requirements are met. 


    New Health Requirements
    (1) K-12 students must show proof of two (2) doses of the measles vaccine, rubella vaccine, and mumps vaccine
    (2) Students entering Kindergarten, 6th and 9th will need to show proof of two (2) doses of the varicella vaccine.
    (3) Pre-K students must show proof of the vaccine against the invasive pneumococcal disease.
